Requirements Survey Form

Members of the Class of 2009:

If you believe that you have or will satisfy the requirements for graduation with the J.D. Certificate in Intellectual Property and Technology Law, you must fill out and sign our Requirements Survey Form (downloadable below) and submit it in hard copy form to Ms. Jaime Horensky in Room 514C no later than April 24, 2009. If you will be relying on grades for classes that you are currently taking this semester, fill out the form listing those classes and we will obtain your grades from the Registrar when they become available.
