Requirements Survey Form

With graduation approaching, I need your help in determining whether you will receive the Intellectual Property (IP) and Technology Law Certificate.

As soon as possible, but not later than Friday, April 22, 2011, please complete the form below and return it to my secretary, Ms. Denise Clemente, in Room 514C. You must submit a hard copy of your signed, completed form (no e-mails will be accepted).

As you know, receiving the IP/Tech Law Certificate requires completion of a range of coursework and satisfaction of certain grade point average thresholds. Those requirements are detailed in the “Requirements Document” posted on the IP/Tech Law Certificate website at <http://www.law.pitt.edu/files/attached/page/IP_Reqs_2010.pdf>. If you are unsure whether you have met all of the requirements, use your best efforts to fill out the form below, and we will check your record against the requirements. For each required category, and within the limits specified by the Certificate requirements, you may rely on whatever courses you choose.

If you are taking courses during the current Spring 2011 semester that you wish to be considered for purposes of the Certificate, leave the “Course Grade” column for those courses blank (we will obtain your grade from the Registrar once it has been reported by your instructor).

In order to graduate with the IP/Tech Law Certificate, you must (1) complete, sign, date, and return this form in hard copy form no later than April 22d to Ms. Denise Clemente in room 514C; and (2) complete and return any other paperwork specified by the Registrar. Submission of this form does NOT substitute for the Registrar’s requirements.
