All graduates are required to submit the on-line Questionnaire below. Information contained in this document is used to prepare the Commencement Program. The form is designed to be accepted only when all fields are completed. You will receive immediate confirmation that the form has been submitted correctly. The deadline to submit the form is April 5.

Commencement Questionnaire Follow-up Requirement
Be sure to check back to this page to verify your name for the Commencement Program anytime between April 8 and April 19.
Please check your listing and make sure that it is correct. If you need to make a correction or change, please contact Kevin Deasy, Associate Dean of Students (email@example.com).
A hard copy of the program will be available at the receptionist counter on the second floor from April 22 to April 26. You will need to stop by and review the information; either approve by initialing or indicate the changes to be made.
